Our BlueCare®65 plan is now called Medicare HMO Blue. You'll still get the same service and quality of care, and can visit the same doctor.
Medicare HMO Blue (Blue Care®65) is one of our Medicare Advantage plans. This plan provides all of your Original Medicare benefits plus additional benefits that Medicare doesn't generally cover. These services include preventive dental care, routine physicals, routine hearing exams, eye exams and allowances toward hearing aids and eyewear. There is practically no paperwork and you're covered anytime, anywhere.
Medicare HMO Blue (BlueCare®65) now also offers two options for prescription coverage—basic and enhanced. You can choose to keep a prescription coverage option, or you can enroll in Medicare HMO Blue without prescription coverage.
Medicare HMO Blue (Blue Care®65) may be right for you if...
In order to join Medicare HMO Blue (Blue Care®65), you must have Medicare Part A and Part B, and continue to pay your Medicare Part B premium. A monthly plan premium and copayments apply. Most Medicare beneficiaries are eligible, including those under the age of 65 entitled to Medicare on the basis of a disability.
If you are a Medicare HMO Blue (Blue Care®65) member, you will use a primary care physician (PCP) who is part of our Blue Care®65 network. Your PCP will coordinate your care and refer you to an in-network specialist, if necessary. The health providers in our network can change at any time. To learn whether your doctor is part of our network, call our Physician Selection Service at 1-800-821-1388. You may also visit our main website at www.bluecrossma.com and use our Find a Doctor feature to locate a physician.
If you choose to go to a doctor outside of our network, you may have to pay the full cost for these services yourself. If you go out of the network, your Blue Cross Blue Shield of Massachusetts plan or Original Medicare plan will only pay for:
The service area for this plan includes: Barnstable, Bristol, Essex, Franklin, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester Counties, Massachusetts. You must live in one of these places to join the plan.
Medicare HMO Blue offers you an optional pharmacy benefit. It's completely voluntary. If you decide you do want to enroll in our prescription coverage, it's important to know that Medicare HMO Blue (Blue Care®65) uses a pharmacy formulary. A formulary is a preferred list of medications selected to meet patient needs. Not all medications are covered under a formulary. Periodically, we may make changes to the covered medications on our formulary. If we remove a medication from the formularly, you will be notified, in writing, before the change is made. Please note: Medicare HMO Blue (Blue Care®65) does cover Medicare Part B prescription medications.
Medicare Advantage plans like Medicare HMO Blue (Blue Care®65) are contracts between the federal government and managed care organizations and are valid for one year. Therefore, the benefits, premiums, copayments, and service areas offered by Medicare HMO Blue (Blue Care®65) may change on an annual basis.
Our contract may be terminated by the federal government, however, in such an event, we will provide you ample notice and ensure that your Medicare benefits continue through another health plan.
